- To provide treatment and advice as appropriate.
- To work with patients in achieving realistic goals.
- To support and advise other health care professionals.
St Barnabas Hospice operates an open referral policy that means that any of the following may refer:
- Support Sister/Macmillan Nurses within the Trust's catchment area.
- GP or other members of the Primary Health Care teams.
- Members of the hospital multi-disciplinary team.
- Self Referral by contacting the Lincoln Day Hospice.
- The day hospice service of St Barnabas Hospice Trust.
- The In-patient Unit or Hospice at Home team of the St Barnabas Hospice Trust.
