St Barnabas Day Therapy service provides palliative rehabilitation, holistic support and promotion of wellbeing to people who have a life limiting condition.
It aims to provide holistic care through an individually tailored, evidence based treatment programme which has clearly defined patient goals and measurable outcomes.
The service will support patients to make informed choices and shared decisions relating to their wellbeing and future care.
We work in partnership with other care providers, specialist support groups andvoluntary organisations.
Referrals are accepted from any health or social care professional, any person with a life limiting condition or their carer / relative.
The referred person must be registered with a Lincolnshire G.P., must be aware of and consent to the referral, and have capacity as determined under the Mental Capacity Act 2005.
Referrals can be made by telephone or SystmOne task to either Lincoln or Grantham Day Therapy centre on the above numbers, or in person at any of the Day Therapy locations.
All referrals for clinical / therapeutic interventions are subject to a holistic, multidisciplinary
assessment by a registered health professional member of the Day Therapy team, to determine need for the service. Assessment appointments can be offered at all locations.
The team is comprised of registered nurses, occupational therapists, physiotherapists, health and rehabilitation support workers, co-ordinators, and a number of volunteer roles, including complementary therapists and chaplains.
